Endometriosis Q & A

When the lining of your uterus (endometrial tissue) grows outside of the uterus, it’s known as endometriosis. The endometrial lining can grow on other reproductive organs, such as your ovaries and fallopian tubes, as well as on surrounding areas such as your bladder and bowel.

Endometriosis causes problems because it consists of tissue that’s usually inside your uterus and thickens and sheds monthly. When the endometrial tissue that’s in places other than your uterus acts like this, it can cause swelling, excessive bleeding, and pain when the blood and tissue don’t have an easy way to shed.

Endometriosis is often characterized by pelvic pain, especially around the time of your menstrual cycle. Other symptoms include:

  • Long-term pain in your lower back and pelvis
  • Bleeding or spotting between periods
  • Infertility
  • Pain during sexual intercourse or afterward
  • Pain during bowel movements or urination

Some women also have digestive distress with endometriosis. This occurs when endometrial tissue is located on your rectum or bowel.

Dr. Gray or Dr. Maslyak reviews your symptoms and medical history to inform their diagnosis. They’ll also perform a pelvic exam, in which they feel for any large swaths of endometrial tissue. Your OB/GYN may have you undergo imaging tests as well, which can help rule out other issues that could be causing your symptoms, such as uterine fibroids or ovarian cysts.

They may also recommend you go on hormonal birth control pills or gonadotropin-releasing hormone agonists (GnRH) — either oral or injectable — to evaluate how your symptoms react. If your pain and excessive bleeding eases, it’s likely you have endometriosis.

Laparoscopic surgery is the only way to definitively diagnose endometriosis. During this minimally invasive procedure, small incisions allow room for tiny instruments that help Dr. Gray or Dr. Maslyak get a small sample of endometrial tissue that can be sent to a lab for testing.

Endometriosis can be managed, but not cured. Birth control pills and GnRH offer management of symptoms. If you desire to get pregnant, you may take GnRH to slow the growth of endometriosis, but it does put you into temporary menopause. Once you stop taking the medicine, you may have a better chance of getting pregnant.

Dr. Gray or Dr. Maslyak may also recommend laparoscopic surgery to remove any trace of endometriosis that could be interfering with your reproductive organs and causing symptoms such as infertility.
