Getting Ready for Your Visit
What to Bring With You
We ask that you bring your insurance card and photo ID with you to every visit. If you have previous medical records, please bring them as well or have them mailed to our office. Your provider will also need a list of all your current medications including the dose and the times you take them.
Your Registration Process
Please plan to arrive to your appointment 15 minutes early to complete the registration process. You may download our registration forms below. Please complete them and bring them with you.These forms are also available in the office. You will also be asked to sign our HIPAA Acknowledgement, as required by law, so that you are aware of our privacy policies.
Your Payment
Please be prepared to pay your deductibles, co-pays, co-insurance or any other amounts due at the time of your visit. We accept a variety of payment options including Cash, Check, MasterCard, Visa or Discover. Please contact our billing office should you have any financial questions or concerns at (561) 368-3775.
Forms & Policies
Before your appointment with us, you can quickly and conveniently access our patient forms by login in to your patient portal. If you have not created a patient portal account yet, you can do so by clicking the login image:
This will allow you to return your forms to us via electronic format in a secure fashion. You will also be able to not only complete your required health questionnaire, but request an appointment, request prescription renewals, pay your bill online, and view lab results online and much more.
You can also access these form by clicking on the links below. This is not the preferred option, because you will have to print the completed forms and bring them in for your visit. The patient portal is generally the preferred method.
